CMS Releases Federal Fiscal Year 2027 Updates to the Health Home Core Quality Measure Sets

The Centers for Medicare & Medicaid Services (CMS) released a State Medicaid Director Letter (SMDL) to announce the 2027 Health Home Core Sets of quality measures for Medicaid individuals with chronic conditions who receive health home services. The 2027 Health Home Core Sets include no new measures or removals for both the 1945 and 1945A health home benefits. This SMDL also includes updated information on the stratification of measures and identification of measures selected for stratification for the 2027 Health Home Core Sets.
