The latest Safer Radiotherapy: biennial error analysis and learning report is now live on the website. It can be viewed here. This report is the ninth in a series of 2-year reports, providing an overview of Radiotherapy Error (RTE) data reported voluntarily from across the UK between January 2024 and December 2025 (n=25,831). The report compares compares data with that from the preceding 2-year period (n=22,113). In addition, the report contains trend analysis of five years of voluntary UK reporting data; January 2021 to December 2025. The report also features anonymised, closed reportable radiation incident data shared by UK inspectorates for IR(ME)R over the same 2-year period. The report affirms that the UK radiotherapy community continues to be invested in practicing in a safe, secure fashion. In addition, changes in reporting volume and data quality between this and the previous 2-year period are discussed, as well as emerging trends identified within the data. We hope this report will provide valuable information that users can employ to continue advancing patient safety in radiotherapy.
