Tips For Submitting Practitioner Applications

Highlighted below are common areas where the Committee have asked for further information or details on new and amendment practitioner licence applications, ensuring this information is included on submission may prevent delays to the resolution of your application.

Practitioner licence applications for all types (diagnostic/therapeutic/research) of procedures:

• Include specific details relating to recent training or experience
• Ensure training for the requested procedure(s) has been completed at the time of submission
• Include details of direct experience for the procedure(s) requested

Practitioner licence applications involving therapeutic procedures:

• Include details of all organs at risk not just the organ to be treated
• Describe appropriate dose limiting measures
• Include information on plans for treatment verification
• For new practitioners, attach a letter of support from a mentor/supervisor (who is a licensed practitioner) 

Letters of support are required for new practitioner applications involving therapy procedures, however they are welcomed and encouraged by the Committee for other practitioner applications also.
